Background

The NHS Long Term Plan (LTP) has included learning disabilities and autism as a key workstream with stated areas of focus including reduction of over-medication through further roll out of the STOMP programme. The national LTP also aims for people to be able to access care closer to home and for healthcare staff working in primary care to have the appropriate training to understand the care needs of people with learning disabilities and / or autism. The HEE mandate specifies that actions must be taken to ensure people are trained to fill the roles needed to deliver the LTP.

Project Aims

The key aim​ of the proposed project is to develop competence and capability in the primary care pharmacy workforce to identify and undertake STOMP / STAMP reviews for people with a learning disability (LD) and/or autism, across the Surrey Heartlands (SH) Health and Care Partnership ICS.

Project Objectives

Objectives​ of proposed project:

To support upskilling of primary care workforce to enable a place-based screening process to identify eligible people with a learning disability (LD) and/or autism, for STOMP / STAMP medication reviews across the Surrey Heartlands (SH) Health and Care Partnership ICS.

To support this implementation by providing access to a specialist pharmacist for advice and guidance.

Expected Outputs

Anticipated achievements:

A training package aimed at primary care workforce developed and delivered to PCN/GP pharmacists working within the Surrey Heartlands (SH) Health and Care Partnership ICS.

To identify people with a learning disability (LD) and/or autism eligible for deprescribing of inappropriate psychotropics prescribed to manage challenging behaviour. This will support understanding the need of the local population within Surrey Heartlands (SH) Health and Care Partnership ICS.
